# When Should Your Child's First Eye Exam Be? A Parent's Guide to Pediatric Eye Care in NYC

# When Should Your Child's First Eye Exam Be? A Parent's Guide to Pediatric Eye Care in NYC

Parents ask me two questions more than any others: "When should I bring my kid in?" and "Doesn't the school check their eyes?" The answers are "earlier than you think" and "not really." Let me explain both.

## The real first-exam timeline \(it's earlier than you think\)

The American Optometric Association recommends three main checkpoints before school age:

- A first comprehensive exam between 6 and 12 months of age
- A second around age 3
- A third before first grade \(around age 5 or 6\)
- Then yearly once school starts

Why so early? A baby can't tell you their vision is blurry. But the first several years of life are when the visual system is wiring itself. If one eye is sending a weaker signal — because it's turned in, because it's significantly more farsighted, or because there's a congenital cataract — the brain will start to ignore that eye. This is called amblyopia, sometimes called "lazy eye." There's a window to treat it effectively. After about age 8 to 10, it becomes much harder to reverse.

I participate in the AOA's InfantSEE program for the 6-12 month visit — a no-cost comprehensive infant eye assessment for families in that age range. Yes, I can examine a baby's eyes. We use lights, toys, and a lot of patience. Most babies find it more interesting than distressing.

## What a pediatric exam looks like by age

The exam changes as the child grows. Here's what to expect at each stage.

**Infants \(6-12 months\):** No chart reading. I use a retinoscope — a handheld instrument that sweeps a stripe of light across the pupil — to measure the eye's refractive error objectively. I don't need a response from the baby to know their prescription. I check that the eyes align, that the pupils react equally, and that the internal structures \(lens, retina, optic nerve\) are healthy. Stereopsis \(3D vision\) testing happens with special targets even at this age.

**Toddlers \(age 3\):** Picture and shape charts replace the retinoscope as the primary acuity measure. The LEA symbols — a circle, square, apple, and house — are designed for kids who don't know letters yet. I check for strabismus \(eye turns\), measure stereopsis more precisely, and start catching amblyopia in earnest. A large NIH-funded study \(VIP-HIP\) found that moderate to high hyperopia at this age is itself a risk factor for amblyopia, strabismus, and reduced school-readiness skills even before any of those show up on a chart, which is part of why this visit matters even for kids who seem to be seeing fine. This is also when farsightedness, astigmatism, and \(less commonly\) nearsightedness become identifiable.

**School-age \(5+\):** Standard letter charts, near-point testing for reading comfort, color vision screening \(often a separate visit if there's a family history\), and a full dilated retinal exam when warranted. By this age, the exam looks a lot like an adult exam, with kid-friendly explanations.

## Signs your child may need glasses

Kids rarely complain about blurry vision because they don't know what "clear" is supposed to look like. The signs are behavioral:

- Squinting to see the board or TV
- Sitting close to screens or holding books inches from the face
- Closing or covering one eye
- Tilting the head to one side
- Losing place while reading, or avoiding reading entirely
- Headaches after school
- Short attention for close work \(sometimes misdiagnosed as ADHD\)

A child can pass a school screening and still have a significant prescription. Hyperopia \(farsightedness\) hides especially well, because kids can focus through it — at the cost of eye strain, headaches, and avoidance of near work.

If your child is also nearsighted and getting worse every year, we should talk about that too. We have ways to slow myopia progression now — low-dose atropine drops, specialty soft contact lenses, ortho-K overnight lenses — that didn't exist a decade ago.

### A note on screens and young eyes

Parents ask me about screen time in almost every visit. Here's what the current evidence actually says: heavy near work \(screens, books, anything held close\) is associated with a higher risk of developing myopia, and with myopia progressing faster once it starts. The mechanism isn't fully settled, but time spent outdoors — at least 90 to 120 minutes a day — appears to be protective, possibly because natural daylight stimulates retinal dopamine in ways that slow eye growth.

So the practical advice isn't "no screens," which isn't realistic. It's "balance close work with outdoor time, and enforce the 20-20-20 rule" — every 20 minutes, your child looks at something 20 feet away for 20 seconds. A kitchen timer on the tablet works better than nagging.

This doesn't replace the exam. But it's the lifestyle piece that supports what we do in the chair.

## Why school screenings miss about a third of vision problems

School screenings are better than nothing, and I'm grateful school nurses do them. But they are screenings, not exams. They check one thing: distance vision.

A typical screening is a 10-foot letter chart. It catches moderate-to-severe myopia. It reliably misses:

- Hyperopia \(farsightedness\) — one of the most common childhood issues
- Astigmatism
- Binocular vision problems \(eyes not working together\)
- Convergence insufficiency \(eyes struggle to turn in for reading\)
- Color vision deficiencies \(about 8% of boys have some form\)
- Eye health issues \(rare in kids, but real\)
- Anything that affects near vision, which is where most learning happens

Published research on school screenings consistently finds substantial miss rates. A 2023 systematic review of home- and school-based amblyopia screening tools found accuracy varies widely by method and age group, reinforcing that a single distance-chart pass doesn't rule out the conditions that actually affect reading and classroom performance. Depending on the study and the screening protocol, roughly a third of vision problems in school-age children go undetected. A child who "passed" their school screening and still complains of headaches, eye strain, or reading difficulty still needs a full exam.

## Comfort-first exams for kids with sensory or developmental needs

A note for parents of kids with autism, ADHD, sensory processing differences, or any anxiety about the exam: I work at your child's pace. There's no script we have to follow.

We can do the exam in pieces across two visits. I can dim the lights, skip the puff-of-air pressure test, and use the soft-touch probe instead. For some kids we lean more heavily on objective tests \(retinoscopy, autorefractor\) that don't require verbal responses. Parents are welcome in the room the entire time, and I'd rather have a child leave half-finished and happy than finish in tears.

One product note worth mentioning: if your child wakes up with crusty, stuck-together lashes, that's often childhood blepharitis — inflammation of the lid margins. A simple lid hygiene routine \(warm compress, then a lid wipe\) usually clears it up. I'll show you how at the visit. Otherwise, pediatric eye care is mostly about the exam itself, not products.
