**Steroid Eye Drops**
In some cases, we may recommend a short course of corticosteroid eye drops to quickly bring inflammation under control before starting a long-term maintenance therapy like cyclosporine. While effective, steroids are generally used for a limited time to avoid potential side effects such as increased eye pressure.
Punctal Plugs: Conserving Your Tears
If your body is producing tears but they are draining away too quickly, punctal plugs may be an excellent solution. Think of your tear ducts as the drain in a sink; if the drain is open, the water flows out. By inserting tiny, biocompatible plugs into the puncta (the small openings in the eyelids that drain tears), we can "close the drain" and keep your natural tears on the eye surface longer. This is a minimally invasive procedure that can be done in our office and provides immediate relief for many patients (National Eye Institute, 2021).
Treating the Oil Glands: In-Office Procedures

**Intense Pulsed Light (IPL)**
Originally used in dermatology, IPL has been adapted to treat dry eye. The light therapy reduces inflammation and abnormal blood vessels around the eyelids while also melting the hardened oils in the meibomian glands. This can significantly improve tear stability.
Autologous Serum Eye Drops
For patients with severe dry eye that does not respond to other treatments, we may recommend autologous serum eye drops. These are created from your own blood, which is processed to extract the serum. Because serum contains essential growth factors and vitamins, these drops closely mimic natural tears and can promote healing of the ocular surface. This is often considered a "last resort" but is highly effective for severe cases.
